How to submit:
OPTION 1. Print and accomplish the form. If you’re from the UPLB area, you may go inside the UPLB Campus, look for the Coop Canteen (behind the Humanities Building), and beside it is the sorority/fraternity (Beta Sigma) tambayan. Before going there, kindly inform NERI or ROCEL that you are on your way so that you will be advised as to who will be available to receive your submission. Once there, you may submit your form along with your payment and wait for an acknowledgment receipt. Send a message to NERI or ROCEL informing her of your drop-off and payment to confirm.
*For your security, we advise you to check The Sorority page to familiarize yourself with the Sorority sisters. We would want to avoid any unauthorized transactions due to misrepresentation/mis-identity.
OPTION 2. If you live remotely from UPLB and cannot be physically available to send the form, you may send us a scanned copy of the printed/completed form, and along with your scanned bank deposit receipt, kindly send them to roadrage@upsigmabeta.com.
For entries, please write SUBJECT: REGISTRATION on the subject line.
For sponsors, please write SUBJECT: SPONSORSHIP on the subject line.
We will be providing you with the receipt once payment is received.
